Summary
Investigation of an oil pollution incident at Eugene Island 32 F platform. Wire lodged in a water dump valve held it open, while scale and corrosion impaired another valve. Platform shutdown removed the sump pump’s instrument gas supply. Repairs restored production, and MMS recommended regular valve maintenance.
