Summary
BSEE investigates hydrocarbon discharges from the GI 43 AP production platform into the Gulf of Mexico. Findings identify sludge and chemical flocking, collapsed skim-tank packing, sump-pump failure and bypassed safety devices. The report also examines delayed remediation and production pressure undermining stop-work authority. No injuries were reported.
