Summary
BSEE investigates several pollution events at West Delta 73 A Platform during March 2019. Produced-water discharge, failed pneumatic pumps and sump overflow released hydrocarbons into Gulf of Mexico waters. The report examines leaking valves, condensate and debris, bypassed safety devices, pump installation, operating procedures and corrective actions. No injuries were reported.
