Summary
The investigation describes a release of 15 barrels of synthetic-base mud after a BOP test on the Green Canyon 158 A-7 well. A butterfly valve was only partly closed, diverting excess mud overboard. The report identifies unfamiliarity with the role and rig, and inadequate communication between the driller and shaker hand.
