Summary
Investigates a tubing lift during well abandonment on Fieldwood’s ST. 205 B platform. Incorrectly sized elevators failed to retain the pipe, which fell approximately 15 feet and struck a worker’s head. Findings address load positioning, omitted elevator-size discussion during job safety analysis and failure to follow elevator-removal procedures.
