Summary
BSEE investigated a falling hammer-union pump connection during well abandonment at West Cameron 61A. The connection struck and injured a worker. A secured reenactment examined sling interaction with the rotating tubing joint. Findings identified inadequate securing, inaccurate shift handover information, deficient supervision and job safety analysis, and unused stop-work authority.
