Summary
This investigation examines a finger fracture during drill-pipe stabbing on Ensco 75. A floor hand guiding pipe with rope sustained a pinch-point injury between a pipe tool joint and pup joint. Findings highlight unused hands-free tools, requirements in the job safety analysis and failure to exercise stop-work authority.
