Summary
BSEE investigates a hand injury during elevator installation on the Enterprise 263 jack-up rig at High Island Block A 469. Rig sway allowed elevators to roll out of the bails, pinching the worker’s left hand. Findings address deficient risk assessment and job safety analysis, unused stop-work authority, glove selection and hand placement.
