Summary
BSEE investigates a fingertip amputation during relocation of Hawse pipe aboard Deepwater Invictus using chain falls. An unsecured pipe rolled from a storage rack after lifting gear was removed. Findings address rack design, inadequate hazard analysis and absent written procedures, with a recommendation to evaluate Transocean’s Hazard ID Prompt card.
