Summary

During platform removal at MU 762A, a polypropylene rope broke while lowering a 190 lb explosive charge into a jacket leg, injuring a worker’s hands and wrist. The investigation identified handling without a tagline, unsuitable rope routing and an inoperative crane swing brake. Work was completed using taglines, a sheave and a snatch block.
