Summary

A safety alert describes failure of a temporary cable spool support during cable installation to a derrick gin pole. The supporting pipe slipped off the rack, allowing the spool to fall and trap an employee’s finger between the pipe’s pad eye and the rack, causing partial amputation of the left little finger.
