Summary
BSEE investigated a finger injury during hydraulic workover operations on Shell’s Salsa platform. A falling elevator link trapped the worker’s finger against a pneumatic back saver. Findings identified improper hand placement, movement of the swivel becket with its pin locked open, and deficiencies in operating procedures and task hazard analysis.
