Summary

An investigation records a severed fingertip during removal of a corroded chain fall and trolley at Boomvang Spar. The load shifted while the worker used his hands. Findings identify improper hand placement, an unrecognised pinch point and inadequate task analysis, and state that a pry bar and two come-alongs should have been used.
