Summary

BSEE investigates a dropped riser joint during crane operations aboard the Diamond Ocean Blackhawk drillship. A worn hydraulic valve falsely indicated latch engagement, allowing an unsafe lift. Findings identify incomplete procedures and hazard analysis, inadequate flagger training, and unreliable visual latch indicators. The district made no recommendations at that time.
