Summary

Investigates a welder’s fatal fall during pollution-pan modification on an offshore rig. The panel found that straps alone supported the pan and that its attachment had not been checked before cutting. Analysis examines absent fall protection, incomplete joint task planning, supervisory misunderstandings and ineffective stop-work communication.
