Summary
BSEE investigates a worker’s leg laceration inside a pressurised welding enclosure on Ursa platform. The worker slipped and fell 12–18 inches, contacting a jagged I-beam edge; his fall protection did not deploy. Findings identify inadequate supervision and hazard assessment, followed by stop-work action, sharp-edge mitigation and revision of the job safety analysis.
