Summary
Investigation of crane damage during boom positioning before a load was attached at Vermillion 261-A-Aux. A failed boom hoist limit device allowed contact with the boom stops. Findings examine a loose, bent extension shaft, an obscured angle indicator, low lighting and hazards missed during job safety analysis and pre-use inspection.
