Summary
BSEE investigated crane boom damage during boat cargo transfers at Green Canyon Block 52 A. An unused stinger caught on boom lacing and broke a weld as the boom was lowered. Findings address inadequate pre-use checks and job safety analysis, ignored supervisory warnings, and continued operation after damage was identified.
