Summary
BSEE investigates a floorhand’s finger injury while moving a mud bucket with an air hoist on an offshore rig. Lowering trapped his finger between the bucket handle and a snubbing post, leading to fingertip amputation. Findings identify missing signalling arrangements, incomplete job safety analysis and failure to follow hands-free lifting procedures.
