Summary
Investigation of a manual master valve failure during temporary well abandonment at EI 252-G. An ejected valve stem struck a contractor’s hand, fracturing his thumb tip. Personnel were evacuated because well stability was unknown. Findings address a missing retainer nut, valve servicing, contractor oversight and management of change.
