Summary
A meter technician repairing a high-pressure gas meter mistakenly removed an assembly from a section retaining 1000 psi. Released gas expelled the gear shaft and pinion, striking him and knocking him unconscious. The investigation identified failure to verify isolation and depressurisation, unclear communication and inadequate supervision.
