Summary
BSEE investigates injuries to two roustabouts during barite transfer from a supply boat to rig storage tanks. A hose disconnected while pressurised struck both workers, who were medically evacuated. Findings identify inadequate supervision, poor communication and omission of a job safety analysis because the task was considered routine.
