Summary
Investigation of a contractor’s fractured tibia during removal of a plumbers plug from a 20-inch process line. Thermal pressure ejected the plug, which struck a support beam and the contractor’s right leg. Findings identify inadequate job safety analysis, failed communication, unread warning labels and hazardous personnel positioning.
