Summary
Investigation of the Triple Play’s collision with Matterhorn Platform while preparing to offload. Both GPS systems failed, resulting in loss of dynamic positioning before the captain regained control. Two caissons were damaged; no injuries, spills or vessel damage were reported. The probable cause was loss of dynamic positioning system power.
