Summary
Investigation of a finger injury during permanent well abandonment at VR-124. A manually handled casing split bowl slid from its jack stand and crushed a worker’s finger. Worn, corroded and debris-filled lifting-eye holes prevented crane removal. Findings also identify uncorrected concerns following stopped operations and unavailable written removal procedures.
