Summary
An investigation examines a gas pipeline riser rupture at WC-45-A during production shutdown. Rising pressure and external corrosion were identified as causes. Findings address inadequate maintenance, inspection and reactivation testing, delayed valve closure and deficient tracking records. Two operators evacuated by boat; ultrasonic testing confirmed extensive metal loss.
