Summary
Investigation of a low-pressure pipe rupture during restart of the Front Runner spar. The release prompted shutdown and evacuation. Internal corrosion was identified as the likely cause, with absent chemical injection quills and inadequately located non-destructive testing contributing. Subsequent inspections led to repairs or replacement of separator inlet piping.
