Summary

An MMS investigation examines the loss overboard of fifteen flat-iron plates during a vessel-to-platform lift. The shifting load slid through and cut nylon slings. Findings address unsuitable slings, single-wrap rigging and apparent training and communication deficiencies. Recommendations cover qualified personnel, verified experience, communication and planning of unusual lifts.
