Summary

Investigates an arm fracture during rental-crane assembly on EC-381-A. A Come-Along chain slipped during boom alignment, moving the suspended section approximately three feet and causing a worker to fall between sections. Findings address attachment of his lanyard to the moving boom, deficient job safety analysis, procedure changes and unused stop-work authority.
