Summary
An investigation examines a dropped swivel pad eye during BOP lifting on Perdido Spar. Contact with a trolley beam sheared its mounting bolt, releasing four pieces near a crew member. Findings identify poor visibility and unaddressed clearance constraints; proposed changes include pad-eye removal verification, safer crew positioning and evaluation of design changes.
