Summary
An investigation examines a floorhand’s injury during back reaming on Hercules Rig 264. Slipping top drive dies and premature compensator movement led to the worker being thrown approximately eight feet into an air hoist. Findings identify incomplete task analysis and maintenance arrangements; corrective actions address inspections, connection verification and weight monitoring.
