Summary
Investigates a hand injury during torch cutting of well casings at the West Cameron 639A decommissioning project. An ungrouted inner casing swayed, trapping the worker’s hand against the outer casing. The report identifies an omitted pinch point in the job safety analysis and notes that welded steel plates could have eliminated it.
