Summary
Investigation of vessel listing and flooding following evacuation for Hurricane Dennis. Ineffective hydraulic isolation allowed ballast and bilge valves to open, enabling water migration. Faulty check valves and failed bulkhead cable transits worsened flooding. Recommendations address isolation procedures, HAZOP review, watertight integrity inspections and remote ballast control.
