Summary
A welder cutting a handrail on the G-Aux platform suffered burns to his right hand and fingers after hot slag penetrated synthetic fuel-gas tubing and ignited its contents. The investigation identifies inadequate isolation, work outside the permitted location and premature glove removal, and records reminders about task-specific analysis and stop-work authority.
