Summary
A mechanic suffered burns during gas-compressor troubleshooting after disconnecting starter exhaust pipework and attempting a fuel-gas start. The investigation identified a possible spark source at the starter bendix and flywheel, inadequate task-specific hazard analysis and failure to stop work when scope changed. Recommendations address safe exhaust discharge and updating JSAs.
