Summary
Investigation of a casing-handling incident at HI 160JA in which shifting bundles trapped a worker’s left foot and ankle on a liftboat deck. Findings identify proximity to the load, inadequate cribbing, unsecured bundles and unused stop-work authority. The worker received hospital assessment and was released that day with no injuries.
