Summary
Investigation of a hydrocarbon release at West Cameron 20-D during return to production after well work. A closed receiving-platform valve left no flow path, overpressurising the departing pipeline and causing gasket failure. Findings identify missing safety-system tests, pre-startup review, regulatory notification and coordination of valve alignment.
