Summary
Investigation of a hydrocarbon leak on Oseberg A during manifold pressure blowdown. A sand-bearing jet downstream of an orifice plate eroded a pipe bend. The report examines tandem-well operation, unsuitable blowdown design, missing inspection coverage, operating documentation and emergency-response weaknesses. Nobody was injured; production stopped for four days.
