Summary
An MMS investigation examines a platform crane lift in which a wireline lubricator fell and struck a contractor. A safety pin prevented the hook latch opening, leaving the sling D-ring on the hook tip. Findings address unsafe rigging, inadequate load securing and supervision; the injured worker was taken to hospital.
