Summary
BSEE investigated a spanner joint lifting incident aboard Noble Stanley Lafosse during completion operations. A nylon sling parted during a tandem topdrive and crane lift, dropping the tailing end approximately six feet without reported injuries. Findings identified missing lift authorisation and planning, unverified sling capacity and unauthorised entry into a temporary drop zone.
