Summary
Investigation of an offshore platform oil release and fire following heater-treater anode mounting failure. An operator sustained minor lacerations after slipping on stairs; personnel evacuated by helicopter. The ignition source remained unknown. The report records sump overload pollution and proposed anode removal, welded blind caps, vessel inspection and hydrostatic testing before return to service.
