Summary
BSEE investigated a subsea mono ethylene glycol release from Na Kika’s EA2 umbilical circuit during 11–15 July 2018. Leaking automatic operated and shutdown valves caused an estimated loss of 17,098 gallons. The report examines delayed detection, pressure alarm behaviour, notification failures and BP’s corrective actions for isolation and monitoring.
