Summary
An investigation documents a generator-building explosion and fire during start-up at South Pass Block 60, C Platform. A separated starter-gas vent line released natural gas indoors. Findings include bypassed gas-detection shutdown, inadequate monitoring, exposed wiring and an inoperable ventilation fan. Building and turbine-intake damage occurred without injuries or pollution.
