Summary

An investigation examines a service loop falling approximately 152 feet to the drill floor during bottom-hole assembly recovery on Diamond Ocean BlackLion. A ring snagged a protruding derrick bolt, pulling the loop from the top drive. Findings identify unsecured rings, inadequate installation guidance, missed manufacturer information and weather-induced swaying. Nobody was injured.
