Summary
An investigation examines a crane personnel transfer in which a sheared swing gear shaft caused loss of swing control and the basket struck a platform catwalk. One employee fell 60 ft into the water. Wind contributed after shaft failure; required inspections had been completed, and no lost-time injuries were reported.
