Summary
A personnel basket carrying four people dropped during transfer from an FPSO towards a barge. Evidence suggested incorrect hook attachment; horizontal loading broke the swivel mechanism. Three passengers fell into the sea and sustained extensive injuries. Actions addressed rigging competence, checks, conspicuous master-ring marking and minimising basket use where safer alternatives exist.
