Summary
BSEE investigates a discharge of 260 barrels of 15 ppg synthetic oil-based mud during pit cleaning and fluid transfers on Stena Forth. Findings identify valve and actuator failure, poor communication and unattended pit monitoring despite permit requirements. The report records contributing failures to check and recognise changing pit levels.
