Summary
A floor-hand fractured his right arm while a torpedo guide was being removed on Helix Q-4000 after abandonment operations. Its unsecured door trapped his arm during lowering with air hoists and a sandline crane. The investigation identified incomplete removal instructions and recorded company measures addressing task analysis, door securing and pinch-point awareness.
