Summary
A welder suffered fatal crushing injuries during casing installation on an offshore drilling rig. A pad-eye caught on the floor edge during crane lifting; further raising caused the casing to swing into him. The operator’s measures include clearing personnel until landing, unequal-length slings and continued use of tag lines.
