Summary
BSEE investigates a flash fire during torch removal of skid pads at West Cameron Block 96 platform B. Residual condensate released combustible vapours through a loosely secured water-skimmer manway. Falling slag and sparks ignited the gas. Findings examine incomplete cleaning procedures, permit approval without walkthroughs, omitted gas testing and misplaced protective tarps.
